About the service

The integrated Health and Social Care Community Mental Health Teams provide mental health and social care services to adults across North Staffordshire.

The services provided are based upon a recovery orientated model which enables adults with mental health related issues and their families, friends and significant others to live and maintain their optimum social roles.

Assessment , treatment and care is provided through a process known as Care Coordination and each person using services will be appointed a Care Coordinator. Services provided include:

  • Advice
  • Support
  • Assessment of mental health and social care needs
  • Treatment and care for people and their carers who have identified mental health needs which cannot be met in primary care.

The service is for adults of working age (16-65) with mental health-related difficulties.
