About the service

The function of the Community Learning Disability Health Team (CLDHT) is to carry out multi-agency and specialist assessments as required which will enable care managers to design packages of services tailored to meet the assessed needs of individuals.

The service provides a range of professional services in order to meet individuals specialist needs. This includes:

  • Advice
  • Health facilitation
  • Treatment
  • Intervention
  • Support
  • Counselling
  • Teaching
  • Educational programmes
  • Evaluation of care

The key areas in which health team members provide the above services are related to:

  • Challenging behaviour
  • Autistic Spectrum Disorder
  • Mental health/forensic/psycho-sexual needs
  • Speech, language and communication difficulties
  • Assisting individuals to access specialist services
  • Assisting individuals to access generic health care services
  • Epilepsy
  • Profound physical disabilities
  • The promotion of continence
  • Health related counselling
  • Health promotion
  • Sensory impairments
  • Feeding/eating difficulties

This list of services is not exhaustive but illustrates the specialist nature of the work of health care professionals in the community

Who is eligible for this service?

Multidisciplinary CLDHTs provide a range of peripatetic / domiciliary support services to assist individuals with a learning disability to live within community settings.
