About the service

The Intensive Support Team is a specialist multi-professional health and social care team dedicated to providing assessment, support and treatment to adults who have a learning disability and complex needs including severe challenging behaviour, autism and mental health needs.

It supports people who are reaching crisis and may otherwise require an admission to hospital.

In addition, it provides support to people during the transition period from assessment and treatment inpatient service to the community home/setting.

The team will provide this care where it is most appropriate for the individual, developing an action plan that will provide a sustainable long term solution.
