The team at Hilda Johnson House works with people with longer-term rehabilitation needs to support them to function safely in the community. They also help to maintain and improve their wellbeing, and prevent hospital readmissions.
The service provides safe, supported accommodation to adults with mental health needs. Hilda Johnson House has space to accommodate eight residents. Service users here will have access to staff support to relearn essential skills to help them live more independently in the future. The cost of this is met by CCG while the trust provides the staffing support.
The interventions offered by the service are delivered in a variety of settings within the community to those aged 18 and above at the time of referral. The team aims to promote wellbeing, independence and recovery and to support adults with mental health needs to develop and maintain their independence and wellbeing within their own accommodation in the local community.
The service has support from staff available 24 hours a day, 7 days a week by telephone if staff are not present on site.
This also includes working with people in the community who have longer-term rehab needs to support them to function safely in the community, help maintain and improve their wellbeing and prevent hospital readmissions. Working closely with the rest of the rehab pathway means that service users can be stepped up or down depending on changing needs.
To be referred, you will need to already be involved with secondary services.
