About the service

The main aim of the service is to provide support to GP practices in treating dementia suffers closer to their own homes. By doing so, the service acts as a bridge between primary healthcare and secondary mental health care.

Location

The Eaves at Marrow House, Dylan Road, off Forrester St, Meir Hay, Stoke on Trent, ST3 1SQ. Satellite sites also at Oak Priory and West End Village
