Older People’s Outreach Team

About the service

The Older People’s Outreach Team supports care being delivered and co-ordinated by community mental health teams to prevent crisis. The team supports older people in their place of residence and offers an alternative to inpatient admission.

If an inpatient stay has been necessary, the outreach team can support a timely discharge. The team works closely with the family/carer of the patient to support a robust care package, which includes carers assessments and advice on behaviours and medication.
