The Crisis Resolution Home Treatment Team is based within our Crisis Care Centre. The Crisis Care Centre is an essential component of the urgent care pathway.
The team operates 24/7 and offers an immediate response for individuals experiencing a mental health crisis, either by telephone or in person, to ensure individuals are signposted to the most appropriate pathway to meet their needs.
This is alongside a home treatment function which support individuals requiring intensive nursing support in the community to help manage and recover from a mental health crisis.
The Single Point of Access Service operates Monday to Friday 9am to 5pm and is responsible for receiving and screening all referrals requiring a mental health service. Referrals can be made from other professionals, family and friends or via self-referral routes, including our Combined Wellbeing Portal.
