About the service

This service operates from a base located in Hanley, covering the whole of Stoke-on-Trent and North Staffordshire for adults with learning disabilities; it consists of a multi-disciplinary team which includes community nurses, occupational therapists, physiotherapists, psychologists, psychiatrists, speech language therapists, dietetics, enablement workers, experts by experience and a health care facilitation team.

Outcomes are identified and agreed with the individual and their carers where appropriate and are delivered using a planned care approach either through the use of a person-centred approach. the team works in partnership with other local agencies and aims to provide input as appropriate in order to address identified needs after the completion of a specialist assessment.

Who is eligible?

Multidisciplinary CLDHTs provide a range of peripatetic/domiciliary support services to assist individuals with a diagnosed learning disability. The service is available to those aged 18 and above.
