The Complex Care Team (formerly Project 86) works in partnership with MPFT and holds the responsibility for commissioning out-of-area Level 2 rehabilitation hospital placements for patients from Staffordshire on behalf of the Integrated Care Board (ICB).
The team provides robust oversight of quality and standards across all placements, ensuring safe, effective, and person-centred care.
The Complex Care Team are committed to ensuring all placements are purposeful, recovery-focused, and time-limited. Working closely with local rehabilitation services and liaising with partner agencies, to promote a coordinated approach that supports continuity of care.
The team’s priority is to facilitate timely discharge, enabling individuals to return to their local area—or locality of choice—with appropriate support in place to sustain recovery and independence.
The Complex Care Team sits within the Rehabilitation Pathway, working collaboratively alongside Summers View, Hilda Johnson House, the Community Rehabilitation Team and the Forensic Liaison Partnership Team.
