About the service
The Adult Community Mental Health Team at the Greenfields Centre provides outpatient, CPN (Community Psychiatric Nurse) services and community day services through a range of multi-disciplinary and multi-agency staff.
The team aims to improve the mental health of the local population. The team incorporates the specialist functions of rehab community function and assertive outreach i.e. people who are difficult to engage with through having severe mental health problems.
The services provided are based upon a recovery-orientated model which enables adults with mental health issues and their families, friends and significant others to live and maintain their optimum social roles.
Assessment, treatment and care is provided through a process known as care coordination and each person using services will be appointed a care coordinator. Services provided include:
- advice
- support
- assessment of mental health and social care needs
- treatment and care for people and their carers who have identified mental health needs which cannot be met in primary care
The service is for adults of working age (16 to 65) with mental health difficulties.
