The Transforming Care Programme’s main function is to discharge people from hospital. The team manages Care, Education and Treatment Reviews (CETRs) to support people in their own homes when at risk of ‘admission to hospital.
The team reviews the quality of care, treatment and support for people either in hospital or recently discharged from hospital.
The team works remotely.
