Service Users and Carers Survey – Experiences During COVID-19

North Staffordshire Combined Healthcare NHS Trust is collecting information to provide a clear view of the experiences of our service users and carers during the COVID-19 global pandemic, and for you to have an opportunity to be involved and influence how we move forward as restrictions ease. 

We would really value your input in providing feedback; it is important that as an organisation we have your views about how our services have been for you during the pandemic, so that going forward we know what worked, what didn’t work, and what was OK and not OK for you.  

The deadline for submissions is Monday 6 July 2020.

All surveys are anonymous and there is no requirement for any personal identifying information to be given on the form. Information will be shared in a report format with the Service User and Carer Council, Directorate teams, Executive Team, and form part of an overarching summary report including other feedback to go to Trust Board.

There is no obligation to complete the survey and this will not affect any services and/or care you receive from Combined. 

We want to learn from you and your experiences, and your views are important so that we can learn from what you say.
