About the service

The Primary Care Mental Health Team is based in GP practices across North Staffordshire and Stoke-on-Trent. The team consists of mental health professionals who provide specialist mental health support and can advise GPs and other practitioners in the assessment and treatment of mental health within primary care.

The aim of the service is to:

  • improve early access to mental health information, advice, guidance, signposting, and support to facilitate access to a wide range of community-based support services
  • provide a holistic, person-centred approach to physical and mental health care whilst considering social care needs
  • promote positive mental health and wellbeing within the community
  • support you to focus on your goals and to access your local community services
  • provide brief interventions to support mental health recovery
